Within a rapidly changing healthcare environment, partnerships across sectors are rapidly proliferating to address the social determinants of health of high need complex populations to improve outcomes. A discussion featuring representatives from Nonprofit Finance Fund, Kresge Foundation, Project Open Hand, University of Utah Health Plans, will share perspectives on what it takes to prepare their organizations for partnering in innovative ways across sectors. They will describe critical operational aspects of their successful partnerships, highlighting approaches related to funding & capital, data sharing, workflow, and community engagement. Augmenting these voices, Nonprofit Finance Fund will moderate the panel and bring national perspectives from its recent efforts under
Healthy Outcomes Initiative, a multi-year partnership with Kresge Foundation, providing advice, tools, convenings, knowledge-sharing, and other assistance to advance health/human service integration and a recent national
report that highlights the landscape of health partnerships.
